This form is used when shipping devices for oxygen concentrator repair or general maintenance. Fill in the details for each unit and place the completed form inside your package. Providing this information helps our technicians identify your equipment quickly and begin service without delays.

For non-warranty, patient-owned CPAP machines, the CPAP Traveler Form records the device information, company details, and the issue being reported. Once submitted, you will receive a PDF copy that can be printed and included with the shipment.
